  1. You are permitted to use and modify the Group Lifestyle Balance™ program curriculum subject to the following Creative commons License: Creative Commons – Attribution-NonCommercial-ShareAlike 3.0.
  2. The DPP GLB Curriculum materials may be downloaded, duplicated, transmitted and otherwise distributed for non-commercial educational or research purposes only, provided proper credits are given to the Diabetes Prevention Support Center (DPSC) of the University of Pittsburgh.
  3. Credit and Attribution — you must give appropriate credit, provide a link to the license, and indicate if changes were made. This must be done in any reasonable manner, but not in any way that suggests the University of Pittsburgh endorses your use.
  4. You may remix, transform, or build upon the material, but if you do so, you must distribute the new work or contributions under the same license as this license.
  5. You may not change the name of the DPP GLB program, but you may add your organization’s name. For example:  “Your Organization Program using DPP Group Lifestyle Balance™ Curriculum.”
  6. You may not use these materials to develop your own training program for the DPP GLB and you may not market these materials or represent yourself as a DPP GLB trainer.
  7. You may not use of any of the GLB manual or materials for commercial purposes without the prior written permission or license from the University of Pittsburgh. For further information on commercial use of the GLB manual or materials, contact the University of Pittsburgh’s Office of Technology Management at 412-648-2206.
  8. By providing your contact information, you allow the Diabetes Prevention Support Center of the University of Pittsburgh to contact you in the future to let you know about updates to the curriculum as well as other pertinent information about the GLB program and training.
